- evaluating client needs and designing, coordinating and delivering tailored support, education and training programs
- conducting interviews to determine the scope and seriousness of client challenges
- tracking client outcomes and preparing regular progress updates
- directing clients to suitable organisations for specialised assistance
- identifying community needs and reviewing the availability of services for health, housing, employment, education, and welfare
- collaborating with local groups, service providers, government departments and businesses to raise awareness and address community concerns
- supporting families and delivering care for children and people with disabilities in group homes, care facilities and service centres
- helping young individuals address social, emotional, and financial difficulties
- drafting funding proposals, resource applications and formal reports for government and other stakeholders
Requirements:
- A relevant diploma or higher qualifications with specialisation in community services.
- Experiences within a community/home care/residential care/disability care/youth work setting desired.
- Outstanding interpersonal and communication skills.
- Ability to commute to client homes and different residential facilities.
- Valid driver's license and access to dependable transportation.
- Police Check.
- Up to date with Flu & COVID-19 Vaccinations
