Yahweh care PTY LTD Yahweh Care Pty Ltd is seeking a compassionate and dedicated Community Worker to join our growing team and support people living with disability to achieve greater independence, wellbeing and community participation. Benefits The remuneration is AU$75,000 - AU$85,000 per annum plus superannuation.
Task & responsibilities Assessing clients’ individual circumstances, social needs, goals and barriers to participation, and contributing to the development and implementation of support strategies that promote community inclusion, independence and wellbeing. Observing clients’ circumstances, needs and progress through regular communication and interaction, and providing feedback to assist with the planning and adjustment of support strategies. Supporting clients to develop practical life skills, confidence and capacity to participate more independently in community, recreational, social, education, health and welfare programs.
Assisting clients to identify, access and engage with appropriate community resources, welfare services, support programs and service providers, including facilitating referrals and supporting attendance where required. Implementing community-based support strategies and participation activities that encourage clients to build social connections, strengthen personal wellbeing and engage meaningfully with their local community. Monitoring clients’ wellbeing, behaviour and changing support needs, and reporting concerns, incidents, risks or progress updates to supervisors and relevant support personnel.
Maintaining accurate, timely and confidential progress notes, client records and incident documentation in accordance with organisational policies, NDIS Practice Standards and legislative requirements. Following individual support plans, organisational procedures, safeguarding practices and workplace health and safety requirements to ensure safe and respectful service delivery.
Liaising with clients, families, carers, colleagues and relevant service providers to support continuity of care and assist clients to access additional services where required.
Providing clients with information about available community resources, welfare services, support programs and participation opportunities. Building positive, respectful and professional relationships with clients, families, colleagues and community contacts while promoting a culturally inclusive and person-centred support environment.
Qualification & experience Qualifications
- At least a Diploma in Community Services, Disability, Individual Support, Aged Care, or a related field.
- Experience Minimum one year of relevant work experience in disability support, aged care, community care, personal care, or a similar support worker/community worker role.

Other requirements
- Current NDIS Worker Screening Check or willingness to obtain.
- Current Working With Children Check or willingness to obtain.
- Current First Aid and CPR Certificate or willingness to obtain.
- Valid driver’s licence and access to a reliable vehicle (not essential but preferred).
