Hope and Change is seeking an experienced NDIS Support Coordinator to join our growing team. This role requires understanding of the NDIS, independent caseload management, and proven ability to meet billable KPI and documentation expectations.
As an NDIS Support Coordinator, you will coordinate, implement and monitor NDIS-funded supports for participants, connecting them with disability, health, housing, community and mainstream supports while maintaining accurate records and reports.
