Job Description

- Monday to Friday 2pm to 10pm

/n

- Join one of Australia's fastest growing Aged Care providers

- Provide strategic and operational leadership to ensure high-quality, resident-centred clinical care.

/n

- Oversee development, implementation, and evaluation of individualised care plans aligned with best practice.

/n

- Mentor and support RNs, ENs, and PCWs to strengthen clinical capability and teamwork.

/n

- Coordinate multidisciplinary collaboration with allied health, GPs, hospitals, and families for holistic care.

/n

- Monitor clinical activities and daily routines (including mealtimes) to maintain service excellence.

/n

- Lead effective communication processes such as handovers, clinical huddles, and team meetings.

/n

- Manage complex resident care needs, including specialist referrals, case reviews, and escalation of health changes.

/n

- Drive staff development through onboarding, training identification, coaching, and performance support.

/n

- Oversee quality and risk activities, including audits, incident management, documentation, and improvement initiatives.

/n

- Implement strategies to minimise clinical risks (falls, infections, adverse events)



and promote resident well‐being.

/n

- Coordinate daily workflow, resource allocation, and communication with senior leaders to ensure efficient operations.

About the Candidate

/n

/n

- You will have an innate empathy for our residents and be committed to continually improving the quality of clinical care provided to them.

/n

- Minimum 1 year experience as a Care Coordinator or similar leadership position

/n

- Must be available Monday to Friday 2pm to 10pm

/n

- Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)

/n

- Post graduate qualifications in aged care would be advantageous but not essential.

/n

- Experience and extensive knowledge of Aged Care clinical assessments, care plans,



AN-ACC and accreditation compliance and documentation.

/n

- Engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.

/n

- High level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.

/n

- A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.

/n

- Ability to manage multiple stakeholders and have a balanced and level‐headed approach to emergency and stressful situations that arise.

/n

- Have a valid NDIS Workers Screening Clearance (or ability to apply via How to apply | NDIS).
