About the Role

AvantiCare is seeking a passionate Social Worker to join its growing multidisciplinary team on a part-time basis, commencing at two days per week initially with the opportunity to increase hours as the social work service continues to grow.

This role offers a varied and rewarding caseload across home care, residential aged care and community settings, supporting clients and their families through life's challenges by delivering evidence-based psychosocial interventions, counselling, advocacy and care planning.

- Social Workers with experience in aged care, community, disability or healthcare settings; and

- Early career Social Workers and recent graduates who are eager to develop their skills within a supportive and collaborative environment.

Key Responsibilities

- Conduct comprehensive psychosocial assessments.

- Provide counselling, emotional support, advocacy and crisis intervention to clients and families.

- Develop and implement individualised support, rehabilitation and discharge plans.

- Connect clients with appropriate community services and external supports.

- Collaborate with clients, carers and the multidisciplinary team to achieve meaningful outcomes.

- Participate in case conferences, care planning and interdisciplinary meetings.

- Deliver culturally responsive, person-centred care that promotes dignity, independence and wellbeing.





- Maintain accurate clinical documentation and contribute to continuous quality improvement initiatives.

What We’re Looking For

You'll be someone who enjoys working autonomously while remaining connected to a supportive team and is passionate about making a meaningful difference in people's lives.

You will also have:

- A Bachelor of Social Work (or equivalent qualification).

- Eligibility for or current membership with the Australian Association of Social Workers (AASW).

- Experience in aged care, disability, community or healthcare settings or be a recent graduate with strong clinical placement experience.

- A genuine commitment to person-centred care.

- Strong communication, interpersonal and organisational skills.

- Ability to work independently while collaborating effectively within a multidisciplinary team.

- Current First Aid and CPR certification.

- Current influenza vaccination.

- Current NDIS Worker Screening Check (or willingness to obtain).

- Current National Police Check (or willingness to obtain).

- Current Professional Indemnity Insurance.

- A current driver's licence and access to a reliable motor vehicle.
