About the role

As the Aboriginal and Torres Strait Islander Health Worker Coordinator within the Community Foot Care Hub, you will play a pivotal role in improving health outcomes for Aboriginal and Torres Strait Islander peoples living with foot disease and other high-risk foot conditions. Working as part of a multidisciplinary team, you will provide advanced clinical care, cultural support, health education and care coordination across inpatient, outpatient, community and virtual care settings.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

- Operate at an advanced level within a specialised Aboriginal and Torres Strait Islander health program, working autonomously under broad direction with minimal supervision and high-level clinical decision-making.
- Manage a varied, complex caseload of patients with foot disease and high-risk foot conditions across inpatient and outpatient settings,



contributing as an integral member of the Central Foot Hub and Royal Brisbane & Women's Hospital multidisciplinary team.
- Promote and deliver efficient, effective, clinically relevant, and culturally safe care that improves patient outcomes and aligns with Aboriginal and Torres Strait Islander health priorities.
- Provide high-standard, person-centred support to clients and families to strengthen self-management capabilities, including navigating health systems, accessing community services, and participating in informed decision-making.
- Deliver and coordinate targeted healthcare programs, including clinical assessment, treatment, care planning, maintenance of healthcare systems, and culturally responsive health education tailored to community needs.
