Melbourne City Mission is seeking two full time Case Managers on a maximum term contract (up to 12 months) within the Homes First program to provide intensive goal-directed case management and outreach to people experiencing homelessness with complex needs.
You will work in a multidisciplinary team to support housing stability, develop tailored goals, and ensure a person-centred, trauma-informed approach while coordinating with care teams and external services to remove barriers.
