About the Program
The Wesley Care finder case worker plays a vital role in supporting vulnerable older people who require intensive assistance to navigate and access aged care and related supports.
We provide support to connect participants to relevant programs and services in the community, targeting individuals who face barriers such as:
Isolation or lack of a support person.
Difficulty processing information or making decisions.
Hesitancy to engage with aged care services or institutions.
About the role
The Wesley Care finder supports vulnerable older people who are unable to navigate the aged care system on their own and who do not have someone else to assist them. This is a frontline, community‑based role focused on ensuring older people do not miss out on the care and supports they need.
Working within a person‑centred, outreach‑based model, Care finders provide free, intensive, face‑to‑face support to help clients understand their aged care options, connect with My Aged Care, arrange assessments, choose appropriate providers, and link with community and support services. The role involves building trusted relationships and supporting people experiencing complex barriers such as social isolation, health concerns, cognitive decline, financial hardship, or cultural and language challenges.
Regular travel within office hours is a significant part of this role,
with the Case Worker working across the local PHN region to meet clients in their homes and community settings.

Essential skills/knowledge
Remuneration Diploma or higher in a relevant field (e.g., aged care, community services, social work, allied health, nursing)
Proven experience supporting older people, people with disabilities, or individuals experiencing social disadvantage usingperson-centred,strengths basedpractice
High-levelunderstanding of the My Aged Care system, including assessment pathways, referral processes, and the aged care servicelandscape.
Ability toidentifyclient needs, provideappropriate servicerecommendations, and navigate multiple service systems.
Sound understanding oftrauma-informedcare and culturally safe practice, particularly in community and homebasedenvironments.
Demonstrated commitment to supporting people from diverse backgrounds, including Aboriginal and Torres Strait Islander peoples, Culturally and Linguistically Diverse communities, LGBTIQA+ communities, and rural/remote populations.
Ability to work respectfully, empathetically, and effectively with vulnerable orat-riskindividuals.
Excellent verbal and written communication skills, including active listening and the ability to communicate with clients from diverse backgrounds.
Proven ability to build andmaintainstrong relationships across service providers, networks,
and community organisations.
Demonstrated ability to work both independently and collaboratively within a multidisciplinary team.
High-levelorganisational and administrative skills, includingaccuratedata collection, documentation, and reporting against programKPIs.
Proficiencywith Microsoft Office and experience using case management or client record systems for documentation and workflow management.
Strong attention to detail and commitment tomaintainingdata integrity.
Strong understanding of confidentiality obligations and privacy legislation relating to sensitive client information.
Strong problem-solving skills, including the ability to assess issues,identifysolutions, and takeappropriate actionwithin role boundaries.
Demonstrated resilience, emotional maturity, and the ability to work effectively under pressure and manage competing priorities.
Ability to remain calm, professional, and effective in challenging, distressing, orhigh-pressuresituations‑pressure situations
Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010, Level 5, with capacity for salary packaging opportunities and the provision of a fully maintained motor vehicle available for private use.
