We are a large, well-established network of bulk-billing medical centres, each supporting 20 - 30 GPs. Our mission is to provide exceptional and accessible healthcare, and we proudly support our communities with 100% bulk billing services and extended opening hours over 365 days per year.
We believe that prevention and proactive disease management are key to reducing hospital admissions and easing pressure on Australia’s health system. To support this vision, we are seeking an experienced, self-motivated Chronic Disease Management (CDM) Nurse Contractor to assist patients living with long-term conditions such as diabetes, hypertension, COPD, heart disease, and other chronic health concerns.
About the Role
As a CDM Nurse Contractor, you will play a vital role in helping patients better understand their diagnoses, navigate their care, and make informed decisions about their health. This contractor model means you have autonomy over workflow within agreed hours.
You will work alongside our GPs and clinical teams to deliver high quality, patient centred chronic disease management and preventative health services.
Key Responsibilities
- Operate medical software for documentation, recalls, investigations, and care coordination, Best Practice software experience preferred but not essential
- Identify and confirm patient eligibility for chronic disease services and relevant MBS billing items.
- Conduct comprehensive health Assessments, including Heart Health, 75+, 40-49 Diabetes Risk, 45-49 Chronic Disease Risk, Menopause/Perimenopause, Intellectual Disability, Refugee/Humanitarian,
and Former ADF Health Assessments.
- Develop and coordinate GP Chronic Condition Management Plans (GPCCMPs). Monitor patient progress, conduct reviews, and support proactive preventative health care.
- Provide chronic disease education to support informed decision making and self management.
- Build strong rapport with patients and collaborate effectively with GPs and allied health providers to ensure cohesive, patient-centred care.
- Maintain accurate documentation in line with Medicare, regulatory, and privacy standards.
- Stay current with updates to chronic disease related MBS items and best practice guidelines.
Contractor Requirements
AHPRA-registered RN (or EN with relevant experience).
Demonstrated experience in chronic disease management, care planning, or general practice nursing.
Strong patient education and communication skills.
Ability to work independently and manage a caseload effectively.
ABN.
Skilled indemnity insurance.
- Ability to meet Medicare compliance requirements.
- Responsibility for own tax, superannuation, and insurance obligations.
