The Nurse Practitioner (NP) is an advanced practice nursing role delivering the highest level of autonomous, clinically focused care and clinical leadership across Company Care Services. The NP combines advanced practice with clinical governance and oversight, guiding Nurse/Clinical Case Managers and Clinical Nurse Educators, strengthening quality and risk management, and ensuring every clinical issue, risk or decision has a clear direction and escalation pathway.
Key Responsibilities and Accountabilities
Advanced Clinical Practice
- Provide advanced, autonomous, clinically focused nursing care and expertise to ensure the delivery of safe, evidence-based, person-centred care across Company Care Services.
- Assess using advanced diagnostic capability; formulate diagnoses and differential diagnoses; and manage complete episodes of care as the primary provider or as part of a care team.
- Hold a small, manageable caseload of participants, providing direct advanced clinical case management and modelling best practice for the team.
- Act as the senior clinical contact for participants, families and external stakeholders where complex or sensitive clinical issues arise.
Diagnosis, Prescribing & Therapeutic Interventions
- Order and interpret pathology, radiology and other diagnostic investigations relevant to the participant's presentation.
- Prescribe, adjust and de‑prescribe scheduled medicines and therapeutic regimens within scope, and access the MBS/PBS where applicable and appropriate.
- Plan and implement therapeutic interventions and initiate timely referrals to GPs, specialists, allied health and other providers.
- Practise in accordance with the NMBA Nurse practitioner standards for practice, assessing using diagnostic capability; planning care and engaging others; prescribing and implementing therapeutic interventions; and supporting health systems.
Clinical Leadership & Governance
- Provide clinical leadership across Company Care Services and supervise, mentor and support Nurse/Clinical Case Managers and Clinical Nurse Educators.
- Sign off care plans and complex care/management plans, and provide recommendations on high-risk or deteriorating participants.
- Monitor and contribute to the development of clinical policies, procedures and guidelines to ensure best‑practice standards and compliance with the NDIS Practice Standards and regulatory requirements.
- Lead or delegate internal clinical audits, identifying risks and areas for improvement to strengthen quality and safety in care delivery.
Behaviour Support & Restrictive Practices
- Hold clinical responsibility for identifying and confirming restrictive practices, determining the correct authorisation pathway, and preparing/lodging Short Term Approvals (STA) and Clarification of Purpose of Medication (COPM) for chemical restraint.
- Ensure ODC notification, PRODA reporting and behaviour support plan (interim and comprehensive) engagement are completed within statutory timeframes; report unauthorised or emergency RP use as a reportable incident.
- Ensure every RP, STA, guardianship, BSP and reporting event flows into the organisational Restrictive Practices Register and BSP Register as the single source of truth, aligned to the Disability Services Act 2006 (Qld) and NDIS Rules.
Scope, Authority and Decision-Making
- Practises autonomously within the endorsed Nurse Practitioner scope of clinical practice, and is credentialed with a documented scope of clinical practice covering all clinical work performed.
- Assesses, diagnoses and treats participants; orders and interprets diagnostic investigations; prescribes and de‑prescribes scheduled medicines within scope; and initiates referrals to other health professionals.
- Holds delegated clinical authority for care and management plans, delegation of care, and clinical decision‑making across Care Services, in accordance with NMBA standards and Company policy.
- Uses the Organisational framework to clarify who is Responsible, Accountable, Consulted, Quality, Informed and Tracking for clinical service‑line tasks.
- Makes autonomous clinical decisions within scope; consults the Clinical Director for matters beyond scope, and escalates clinical risk, safeguarding concerns, restrictive practice concerns and reportable incidents through the appropriate pathway.
- Partners with Quality, Safeguarding & Compliance for incidents, restrictive practices, audits, registers and continuous improvement, keeping quality assurance independent of the doing.
Quality, Risk and Compliance
- Identify, elevate and document clinical risk; apply the unified participant tiering model and RAG risk overlay, and elevate deterioration through the correct clinical governance forum.
- Ensure adherence to the NDIS Practice Standards, NDIS Code of Conduct and NMBA standards; participate in audits, reviews and continuous improvement.
- Elevate incidents, complaints, safeguarding issues and restrictive practice concerns through the correct organisational pathways.
People Leadership & Supervision
- Provide clinical supervision, guidance and competency sign‑off to the clinical team, with clear expectations for documentation, escalation and person‑centred delivery.
- Support onboarding, training needs analysis and role clarity, translating EOM expectations into practical daily clinical practice.
- Address clinical performance, conduct or competency concerns promptly and document these in accordance with the Company's People & Culture processes.
Reporting, Data and Continuous Improvement
- Produce and contribute to clinical progress reports, care/management plans and outcome summaries within Company templates, branding and quality standards.
- Capture data and evidence to support participant outcomes and satisfaction, and translate feedback into continuous improvement register items.
- Comply with the continuous improvement process, completing quality improvement forms and promoting the process across the team.

Accountabilities and Framework
- Service Delivery (Clinical Services) Provides advanced, autonomous clinical practice and clinical leadership under the Clinical Director. Leads safe, evidence-based, person‑centred care across Care Services.
- Behaviour Support & Restrictive Practices Accountable for correct restrictive practice (RP) identification, authorisation pathway, STA/COPM, ODC/PRODA reporting and behaviour support engagement across the RP task cards, aligned to the Disability Services Act 2006 (Qld) and NDIS Rules. Escalates unauthorised use.
- Care Services (Clinical Oversight) Accountable for clinical oversight of Care Services delivery, care and management plans, delegation of care, and high‑intensity supports; signs off complex care/management plans and models best practice with a small caseload.
- Clinical Risk Accountable for clinical risk identification, escalation and documentation across the caseload; applies the unified tiering and RAG risk overlay and escalates deteriorating participants through the clinical governance pathway.
- Clinical Intake, Assessment & Care Planning Undertakes advanced clinical assessment (diagnostic capability), formulates and reviews care plans, and confirms tier and RAG rating at intake and review. Prescribes and implements therapeutic interventions within scope.
- MDT Care Leads or contributes to Clinical MDT / Case Conference for high‑complexity participants; provides advanced clinical formulation and least‑restrictive recommendations.
- Clinical Governance Consulted on clinical governance matters. Accountability sits with the Clinical Director. Supports clinical outcome monitoring, audit and standards.
- Restrictive Practices & Behaviour Support Governance Every RP, STA, BSP and report produced flows to the Clause 11 RP Register and BSP Register held by the Head of Quality, Systems & Projects as the single source of truth.
- Incident Management & Safeguarding Consulted on clinical incidents and reportable RP use; hands the baton to further Departments for classification, investigation and closure; kept informed of outcomes.
- People & Culture interface Supervises and mentors Nurse/Clinical Case Managers and Clinical Nurse Educators; supports competency sign‑off, training needs and performance documentation in line with HR processes.

Selection Criteria
Essential Skills, Experience and Attributes
- Current endorsement as a Nurse Practitioner with the Nursing and Midwifery Board of Australia (NMBA) / AHPRA, with a documented scope of clinical practice.
- Current general registration as a Registered Nurse in Australia, with no conditions or undertakings relating to unsatisfactory qualified performance or unprofessional conduct.
- Master's degree qualifying for NP endorsement, and the equivalent of at least three years' (5,000 hours) full‑time experience at an advanced clinical nursing practice level.
- Advanced clinical assessment, diagnostic, prescribing and problem‑solving skills, with the ability to manage complex and challenging clinical situations autonomously.
- Demonstrated experience in disability, community or complex‑care nursing, including high‑intensity supports and delegation of care.
- Strong understanding of the NDIS Practice Standards, restrictive practices authorisation (Disability Services Act 2006 (Qld) / NDIS Rules) and clinical governance.
- Excellent communication, interpersonal and leadership abilities, with a collaborative, person‑centred and culturally safe approach.
- Proficiency in clinical documentation and systems (e.g. Splose, ShiftCare, Microsoft 365).
Desirable
- Postgraduate qualifications or credentialing in a relevant clinical specialty (e.g. disability, rehabilitation, mental health or complex/community care).
- Health Service Management qualification, and a training and assessment (TAE) qualification.
- Experience contributing to an Enterprise Operating Model, SOP library or quality management system.
Other Requirements
- Current Queensland driver's licence.
- A Paid Blue Card, or the ability to obtain one, is essential.
- NDIS Worker Screening Check, or the ability to obtain one, is essential.
- Current right to work in Australia.
Current CPR / First Aid, and willingness to travel between service sites and into the community as required
