Job Description

We are seeking compassionate and motivated Disability Support Workers (Linkers) to join our energetic team at Link to Life. As a Linker, you will provide 1:1 direct support to individuals with psychosocial disabilities, helping them navigate daily challenges and promoting their mental well-being. Your work will involve providing personalised care, assisting with daily activities, and encouraging social inclusion.

Key Responsibilities

- Provide high-quality, person-centred care and support to individuals with psychosocial disabilities




- Assist participants with daily living activities such as personal care, meal preparation, and medication management
- Support participants in engaging with their communities and participating in social and recreational activities
- Implement individualised care plans and support strategies tailored to mental health needs
- Foster a positive and supportive environment that encourages participant independence and well-being
- Monitor and report on participant progress and any changes in their needsCollaborate with other team members and healthcare professionals to ensure comprehensive care

Qualifications

- Certificate III or IV in Disability, Mental Health, Individual Support, or equivalent (desirable but not mandatory)
- Previous experience in disability support, mental health, or a related field (preferred)
- Strong communication and interpersonal skills
- Empathy, patience, and a genuine passion for supporting individuals with psychosocial disabilities
- Ability to work both independently and as part of a team
- Lived experience of disability and/or mental health challenges (highly valued)
- Valid driver’s license and access to reliable transportation
- Current First Aid and CPR certification (or willingness to obtain)
- Ability to obtain a NDIS Worker Screening Check and Working with Children Check
