Our client is a large and highly regarded health service committed to delivering high-quality, recovery-oriented mental health care across community settings. An exciting opportunity exists for an experienced Mental Health Clinician to join a dedicated Community Mental Health Team, supporting consumers living with complex mental health conditions to remain well and connected within their communities. This role offers the opportunity to work as part of a multidisciplinary team delivering evidence-based mental health interventions, care coordination, risk assessment, and recovery planning for consumers across a diverse caseload.
Roles & Responsibilities:
- Undertake comprehensive mental health assessments, risk assessments, and ongoing clinical reviews.
- Develop, implement, and evaluate recovery-oriented care plans in partnership with consumers and their families.
- Provide case management and care coordination for consumers with complex mental health needs.
- Deliver therapeutic interventions, psychosocial support, and relapse prevention strategies.
- Monitor clinical progress and identify early signs of deterioration, escalating concerns appropriately.
- Liaise closely with psychiatrists, GPs, psychologists, social workers, and other community services.
- Maintain accurate clinical documentation and ensure compliance with legislative and professional standards.
- Participate in multidisciplinary meetings, clinical reviews, and quality improvement initiatives.
Requirements:
- Current AHPRA Registration as a Registered Nurse
- Postgraduate Qualification in Mental Health Nursing
- Experience working within Community Mental Health Services
- Strong skills in mental health assessment, risk management, and care planning
- Experience managing complex mental health caseloads
- Robust understanding of recovery-oriented and trauma-informed practice
- Excellent communication, stakeholder engagement, and documentation skills
- Ability to work autonomously while contributing effectively to a multidisciplinary team
