Nurse Practitioner - Aged Care - Traralgon and Paynesville

- Flexible Weekday Scheduling – Choose one regular weekday per week in Traralgon, one regular weekday per fortnight in Paynesville, or work across both locations to suit your existing commitments.

- No After-Hours, On-Call or Weekend Requirements – Focus on resident care without the demands of after-hours,
on-call or weekend work.

- Travel reimbuirsed

- Travel Support – Reimbursement is available for eligible travel, subject to company guidelines.

The Role
/n
You will provide advanced assessment, diagnosis, prescribing, treatment and care planning for residents within Residential Aged Care Homes. You will work within a collaborative multidisciplinary model alongside on-site nurses, General Practitioners, telehealth Geriatricians and Physicians, with dedicated administrative and operational support so your focus remains on delivering high-quality clinical care.

Opportunities are available across:
/n
/n
- Traralgon – one regular weekday per week, 9AM - 5PM
/n
- Paynesville – one regular weekday per fortnight, 9AM - 5PM
/n/n
Applications are welcomed from Nurse Practitioners interested in either location individually or across both locations.
/n
What You'll Bring
/n
/n
- Experience in aged care, gerontology, palliative care or chronic disease management highly regarded
/n
- Advanced clinical experience in assessment, diagnosis, treatment and health promotion
/n
- Strong clinical reasoning, communication and leadership skills
/n
- Experience in quality improvement and evidence-based practice
/n
- Sound knowledge of aged care legislation and regulatory requirements
/n
- A collaborative, resident-centred approach to care
/n
- Comfortable using technology and clinical software systems
/n
- Current driver's licence, reliable vehicle and professional indemnity insurance
/n/n
Qualifications
/n
/n
- Master of Nursing (Nurse Practitioner)
/n
- Current AHPRA registration as a Registered Nurse with Nurse Practitioner endorsement
