Nextt is looking for two experienced frontline leaders in disability and complex care to join our Client Experience team in Melbourne. These roles have been created to support individuals transitioning from hospital settings into safe, supported community living.

This is a unique opportunity to play a critical leadership role supporting people with mental health, high-intensity, and complex clinical needs to rebuild independence and quality of life.

Portfolio

Melbourne North / North West - Port Melbourne, Footscray to Melton and surrounding suburbs - Need to travel to Ballarat one day per week

We operate a hybrid working model, combining time in our purpose-built Melbourne CBD office, working from home, and connecting with clients and teams across your region.

This role is ideal for a clinically experienced professional who is ready to step into a strategic leadership role in complex care.

You May Currently Be Working As a

- Registered Nurse (AHPRA registered)
- Behaviour Support Practitioner (BSP)
- Allied Health Professional

If you have experience supporting individuals with mental health, high-intensity supports, or complex behaviours, this role offers the opportunity to lead services that make a real difference.

You will oversee secure, structured hospital-to-community transitions, ensuring individuals receive the right supports to thrive in community living.

Key Responsibilities

- Lead a portfolio of clients with complex and high-intensity support needs
- Manage hospital-to-community transitions, ensuring safe and sustainable care arrangements
- Build, mentor and lead high-performing frontline support teams
- Oversee care plans, behaviour support plans, and clinical risk management strategies
- Collaborate with hospital discharge teams, allied health professionals, families, and stakeholders
- Drive quality, compliance, and continuous service improvement
- Balance clinical oversight with effective portfolio management

What You’ll Bring

- A background in Nursing, Behaviour Support, or Allied Health (preferred)




- Strong leadership experience (minimum 2 years managing teams)
- Experience supporting individuals with mental health and/or complex care needs
- Strong knowledge of Positive Behaviour Support and risk management
- Confidence working within multidisciplinary teams
- A collaborative approach to working with families, clinicians and hospital teams
- A passion for delivering high-quality, person-centred services
- Experience within the NDIS and disability sector
