About the Role:

The Support Worker role involves traveling between customers homes and assisting them with daily living supports, access to community as required. Promoting independence to maximize the living, social and recreational opportunities of NDIS participants with disabilities such as physical, intellectual, neurological, acquired brain injury, or psychosocial in the comfort of their home.

We offer a range of services that enables our customers to feel more inclusive, self- dependent and participate within their own community.

Duties and responsibilities may include:

- Personal care, showering dressing grooming and medication assistance.
- Supporting with domestic assistance, light household cleaning, laundry, and meal preparation
- Provide individual social support/ transportation, social activities, shopping groceries and attending medical appointments.
- Supporting customers in the use of aids and personal equipment including standing or walking frames, wheelchairs, and other physical aids
- Monitoring the well- being of customers and reporting on their progress
- Provide accurate and detailed incident reports and progress notes of customer's progress.
- Building genuine rapport with the customers in a meaningful way.

We are looking for:





- Experienced support workers in the field of Disability, Mental Health, or complex care
- Minimum 2+ years of support worker experience is required.
- Passionate Support workers that what to empower customers to achieve their goals.
- Support workers who can work independently and effectively manage own time.
- Social support workers that love interacting in meaningful conversation.
- Workers with a can do attitude who are resilient and reliable.

Requirements:

- Certificate III in individual support or Cert IV in Disability/ Aged Care or equivalent qualification
- Current NSW Driver's License
- Reliable, registered and fully insured vehicle.
- Background checks must be obtained before starting work.
- NDIS Worker Screening Check must be completed before job offer is made.
- Working with Children's Check
- First Aid Certificate
- First Aid Certificate must be obtained within 30 days of commencement.
- Smartphone with internet access
- Must be willing to obtain NDIS required modules and Infection Control Training
- Australian work rights are required (valid work visa accepted).
