Right at Home Gosford are seeking qualified and experienced Clinical Care Partners - EN/RN to help our Aged Care clients live empowered and independent lives in their own homes. Our mission is to improve the quality of life for those we serve.

Our preferred candidate will demonstrate client-centred practice, be a natural relationship builder, helpful, and reliable; making you a great addition to our team of experienced RNs and health professionals.

Your new role will be responsible for:

- Meeting with clients and their families and coordinating their needs within their funding model
- Conducting client care assessments and creating care plans to best meet client requirements
- Handling information requests, feedback, and general enquiries




- Ensuring all documentation is accurately and comprehensively completed in Right at Home databases and software programs
- Attending to clients nursing & wound care and performing health checks to improve their care
- Allocating and training care workers to match clients needs

To be successful in this role, you will need:

- Current AHPRA registration
- Highly developed verbal and written communication skills
- Strong organisational, problem solving and time-management skills
- Current Australian National Police Check
- The right to work in Australia

Preferred Skills

- Sound understanding of the Aged Care and Disability systems
- Demonstrated experience supporting staff
