Respect is seeking a Care Partner – Regional Support to deliver person-centred, enablement-focused case management across multiple regions. Based out of our Launceston office, you will build trusted relationships with participants and families, tailoring care plans to promote independence and wellbeing in their own homes.
You will assess needs, develop goals, coordinate services, and monitor wellbeing while working with a cooperative team to ensure consistent, high-quality care across regions.
