Rockpool Residential Aged Care is a boutique provider, proudly Queensland owned. We believe great careers and great care go hand in hand. Our My Day My Way model of care puts the people we care for at the centre of everything, shaping their days around their choices, not our routines.
This philosophy shapes how we work and what makes a career at Rockpool different: purpose that matters, contemporary leadership that backs you, beautiful up-to-date homes designed for the people in them, smart technology that frees you to focus on people, and room to grow with Queensland's fastest-growing provider.
About the role
Reporting to the General Manager, the Clinical Nurse Consultant (Clinical Care Manager) supports the clinical direction of the home and plays an important role in strengthening our approach to safety, quality and compliance. You'll help shape and maintain effective clinical governance frameworks, contribute to accreditation readiness, and build the clinical capability of the team.
You're also a key point of support for clinical escalation, helping to identify clinical deterioration early, assisting with the management of complex clinical care needs, and contributing to stronger, more reliable clinical systems. This role is centred on governance, guidance and assurance, working in close partnership with Community Leaders (Care Managers) and the General Manager.
Key responsibilities include:
- Serve as the most senior clinical expert on site, providing confident and timely judgement for deteriorating, high-acuity and complex residents
- Lead multidisciplinary case management, bringing together GPs, allied health and specialist services to deliver coordinated, person-centred care
- Partner with Community Leaders and Registered Nurses to assess, plan and review care for residents with complex and changing clinical needs
- Lead AN-ACC assessments across the home, ensuring every classification accurately reflects each resident's true clinical acuity and supports funding integrity
- Proactively monitor AN-ACC classification outcomes and work with the care team to close gaps between assessed needs and funded classifications
- Drive clinical governance at the home level through audits, quality reviews and continuous improvement planning
- Lead accreditation readiness and support ACQSC audit and assessment processes with confidence and rigour
- Coach, mentor and educate Registered Nurses and Community Leaders, building clinical confidence and capability across the workforce
- Partner with the Head of Clinical Governance to implement quality, safety and compliance initiatives at the home level
Skills and experience
- Significant post-registration clinical experience in residential aged care, with demonstrated expertise in complex clinical case management
- Proven expertise in AN-ACC assessment, classification, funding integrity and documentation standards
- Strong knowledge of the Aged Care Act 2024, Aged Care Rules 2025 and the Strengthened Aged Care Quality Standards
- Demonstrated experience in clinical governance, quality improvement and incident management including root cause analysis
- Proven ability to lead and support accreditation readiness and regulatory audit processes
- Experience coaching and developing clinical teams, with the ability to build capability and influence practice without direct line management authority
- Experience engaging with the Aged Care Quality and Safety Commission is highly regarded
- Bachelor of Nursing (or equivalent) and current AHPRA registration
- Postgraduate qualifications in aged care, clinical leadership or a related specialty highly regarded (desirable)
