East Coast Care Connect is seeking an experienced Service Coordinator for our Northern Rivers Team in Recent South Wales. You will coordinate NDIS-funded supports ensuring participant goals, plans and service agreements are met with high-quality, person-centred care.
You will liaise between participants, families, frontline staff and external stakeholders, ensuring responsive service delivery aligned with NDIS Practice Standards and governance.
