About the Role As a Community Worker, you will provide person-centred support to older people living at home. You will assist clients to maintain their independence, wellbeing, and quality of life while delivering services in accordance with the Aged Care Quality Standards and the Support at Home Program.
Key Responsibilities Provide personal care, including showering, dressing, grooming, and toileting assistance. Assist with domestic tasks such as cleaning, laundry, meal preparation, and shopping.
Provide transport and accompany clients to medical appointments, shopping, and community activities. Offer companionship and social support to reduce isolation. Support clients with mobility and transfers using secure manual handling techniques.
Complete progress notes and documentation accurately and promptly. Report changes in clients' health or wellbeing to the Care Manager. Follow individual care plans, organisational policies, and infection control procedures.
