Full-time Family Support Worker position available with competitive salary. Work in a dynamic team in North Strathfield, NSW 2137.
- Full-Time Family Support Worker
- $79,000 – $90,000 p.a. plus superannuation

Position Overview: Astute Living Care is committed to empowering individuals to live independently and improve their quality of life through compassionate and personalised community support services. As a Family Support Worker at Astute Living Care, you will provide practical, emotional and community-based support to participants and their families, helping them achieve their goals, strengthen independence and enhance wellbeing. This position requires empathy, excellent communication skills and a strong commitment to person-centred care.
Responsibilities and Tasks:
- Identify participants and their families' support needs and assist them in accessing appropriate NDIS, mental health, community, and allied health services to promote independence and wellbeing.
- Work collaboratively with Support Coordinators, healthcare professionals, and multidisciplinary teams to develop, implement, and review personalised support plans that reflect participants' individual goals and circumstances.
- Encourage participants to engage in community participation, social inclusion activities, skill development programs, and mentoring opportunities that enhance confidence, independence, and quality of life.
- Provide practical in-home support with daily living activities, personal care, household routines, and life skills while promoting participant choice, dignity, and independence.
- Support participants and their families by accompanying them to appointments, community activities, and service meetings, providing practical assistance, advocacy, and emotional support where required.
- Apply person-centred and recovery-oriented practices when supporting individuals experiencing psychosocial, mental health, or behavioural challenges, ensuring secure, respectful, and compassionate service delivery.
- Foster positive relationships with participants, families, carers, and community stakeholders to strengthen informal support networks and encourage meaningful community engagement.
- Maintain accurate, timely, and confidential records of participant progress, support activities, incident reporting, and service documentation in accordance with NDIS Practice Standards, organisational policies, and legislative requirements.
Qualifications and Skills:
- Relevant qualification.
- Minimum 1 year experience as a Family Support Worker.
- Strong understanding of person-centred, recovery-oriented and community support practices.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within multidisciplinary teams.
- Strong organisational, documentation and time management skills.
Salary: The role offers a remuneration package ranging from $79,000 – $90,000 p.a. plus superannuation for a 38-hour work week.
