We are seeking an experienced Senior Support at Home Coordinator to lead a team of Care Coordinators while managing a portfolio of complex clients across the Support at Home, Home Care Package (HCP) and Commonwealth Home Support Programme (CHSP) programs.

We are seeking an experienced Senior Support at Home Coordinator to lead a team of Care Coordinators while managing a portfolio of complex clients across the Support at Home, Home Care Package (HCP) and Commonwealth Home Support Programme (CHSP) programs.

About the Role

As our Senior Support at Home Coordinator, you will:

- Lead,



mentor and support a team of Support at Home Coordinators to deliver exceptional customer outcomes.

- Manage team performance through regular coaching, one-on-one meetings and performance reviews.

- Monitor and achieve key operational KPIs including:

- Care Management billable hours

- Care Management utilisation

- Client satisfaction

- Timely care plan reviews

- Service commencement timeframes

- Documentation compliance

- Funding utilisation

- Budget performance

- Oversee a portfolio of higher complexity clients and provide escalation support for clinical and service delivery matters.

- Ensure high-quality care planning that reflects client goals, assessed needs and legislative requirements.

- Drive proactive care management to maximise client outcomes while ensuring funding is utilised appropriately.

- Identify opportunities for additional supports, reassessments and funding supplements.

- Work collaboratively with Care Connection, Operations, Rostering, Allied Health and Quality teams to ensure seamless service delivery.

- Monitor compliance with the Aged Care Act 2024, Support at Home Program Manual and Aged Care Quality Standards.

- Analyse team performance data and implement strategies for continuous improvement.

- Support workforce capability through coaching, onboarding and ongoing development.

- Build relationships with hospitals, health professionals, community organisations and referral partners to support business growth.

- Champion a culture focused on customer experience, accountability,



continuous improvement and person-centred care.

About You

You are a proven leader who combines strong operational capability with genuine compassion for older Australians.

You will bring:

- Diploma or higher qualification in Aged Care, Community Services, Nursing, Allied Health or a related discipline.

- Demonstrated leadership experience within aged care.

- Extensive experience across Home Care Packages, Commonwealth Home Support Programme and/or the Support at Home Program.

- Strong understanding of the Aged Care Act 2024, Support at Home reforms and Aged Care Quality Standards.

- Experience managing KPIs, workforce performance and operational targets.

- Excellent coaching and people leadership skills.

- Strong financial acumen with the ability to monitor funding utilisation and care management performance.

- Exceptional organisational, communication and problem‑solving skills.

- Confidence using client management systems, reporting platforms and Microsoft Office.

- Current Driver Licence and willingness to travel within your allocated Aged Care Planning Region.

- National Police Check and NDIS Worker Screening Check (or willingness to obtain).
