About the role

As a Housing Case Manager, you will support individuals and families who are experiencing homelessness or are at risk of homelessness by providing assessment, case management, counselling, advocacy, and referral services. You will work within The Salvation Army’s Specialist Homelessness Services team to deliver holistic, client‑centred support, including assistance through the Private Rental Assistance Program to help prevent and end housing crises. This permanent part‑time position (30.4 hours per week) is based in Horsham, VIC and reports to the Wimmera Team Leader.

Key Responsibilities

- Conduct comprehensive client assessments and provide tailored case management, advocacy and support.
- Develop and implement holistic, strengths‑based case plans and sustainable housing solutions that empower clients to achieve greater independence.
- Support clients to access, secure and maintain appropriate housing, including assisting households to establish successful private‑rental tenancies.
- Coordinate referrals and connect clients with community services, employment pathways, training opportunities and other supports to enhance wellbeing and self‑sufficiency.
- Maintain accurate client records, case notes and reporting requirements while contributing to a collaborative team environment and delivering high‑quality client outcomes.

Qualifications

- Relevant tertiary qualification in Social Work or a related human services field, or equivalent demonstrated experience in case management and client support.
- Demonstrated knowledge of homelessness, including context, causes, issues, philosophies and legislation.




- Ability to apply homelessness models, approaches, theories and practices when dealing with clients.
- Understanding and ability to respond to the needs of individuals experiencing homelessness or at risk of homelessness.
- A current VIC Driver’s Licence.
- A current Employee Working with Children Check.
