Uniting NSW.ACT is seeking a Support Coordinator to assist participants with NDIS plans, navigate portals and establish service agreements across multiple sites. This 9‑month, full-time role involves 38 hours weekly, with the option to work from home twice weekly and travel to North Parramatta, Leichhardt or Campbelltown as required.

You will apply person-centred approaches, liaise with providers and Plan Managers, and help participants access mainstream supports, while maintaining solid
