About the Role

As a Case Manager you will deliver high‑quality case management support services across Bass Coast and South Gippsland to young people, couples and families, including those with children. The role is a permanent, part‑time position (30.4 hours per week) based on‑site in Leongatha, VIC, reporting to the Client Services Coordinator.

Responsibilities

- Deliver high‑quality support services to young people, couples and families using trauma‑informed, youth work and social work frameworks.
- Conduct assessments, develop and implement individual case plans in collaboration with clients, and regularly monitor progress and outcomes.
- Engage with young people who have complex needs (e.g., AOD, mental health, out‑of‑home care, adolescent violence) and connect them with community supports and advocacy where needed.
- Maintain accurate client records, manage risks, and contribute to programme development and service improvements within an integrated team approach.
- Facilitate group activities, life‑skills sessions and therapeutic programmes, and participate in portfolio teams and strategic working groups to support client wellbeing and programme delivery.

Qualifications & Skills

- Completed tertiary qualification in social work, social sciences or a related field (desirable).
- Demonstrated experience in delivering case management practice within a homeless service setting.
- Excellent written and verbal communication skills and adept at using IT.




- Strong organisational and administrative skills with capacity to balance workload, determine priorities and meet deadlines, whilst managing crisis assistance needs.
- Current Victoria Driver’s Licence.
- Current Victoria Employee Working with Children Check.
- Mandatory Nationally Coordinated Criminal History Check.

Conditions & Compensation

Compensation is in accordance with the Social, Community, Home Care and Disability Services Award – Level5.
