About the Role
Grade 3 Social Worker – Team Leader, Transition Care Program (St George’s Hospital, Kew). Provide high‑quality, evidence‑based case management for aged care clients, lead day‑to‑day operational functions, and support multidisciplinary teamwork.
Responsibilities
- Deliver high‑quality, evidence‑based, person‑centred care.
- Lead day‑to‑day operational functions: intake, occupancy management, case reviews, team meetings and staff performance management.
- Participate in staff supervision, professional development and performance review processes.
- Provide expert advice and secondary consultation within your area of expertise.
- Lead quality improvement initiatives, service development and continuous improvement activities.
- Undertake comprehensive assessments using case management principles and strengths‑based approaches.
- Deliver discipline‑specific assessment and intervention in line with best practice.
- Develop collaborative care plans with clients, carers and families.
- Facilitate client‑centred goals that support health, wellbeing and independence.
- Coordinate multidisciplinary and specialist services to meet client needs.
- Monitor client outcomes and coordinate discharge planning, ensuring appropriate community supports are established.
- Complete Transition Care Program assessments, including financial assessments and program orientation.
- Apply advanced clinical decision‑making and risk assessment skills to manage complex client presentations.
- Establish effective working relationships with multidisciplinary teams, community providers and key stakeholders.
- Advocate for clients and families to support seamless navigation through health and aged care services.
- Maintain accurate clinical documentation,
activity reporting and records in accordance with organisational and legislative requirements.
- Manage workload effectively while responding to program priorities and organisational needs.
Selection Criteria
- Essential tertiary qualification in an Allied Health discipline.
- Minimum seven years’ post‑graduate experience in a relevant acute, community health and/or aged care setting.
- Eligibility for membership of the relevant professional association and/or current skilled registration.
- Current Victorian Driver Licence.
- Current National Police Check (or willingness to obtain).

- Highly developed communication, negotiation, organisational and problem‑solving skills.
- Advanced knowledge of case management principles and contemporary healthcare practice.
- Demonstrated expertise in comprehensive client assessment.
- Well‑developed documentation, education and teaching skills.
- Knowledge of community resources and support services.
- Sound understanding of the Aged Care sector and the Transition Care Program.
- Commitment to ongoing professional development.

- Post‑graduate qualification relevant to clinical practice (preferably at Master’s level).
