POSITION DESCRIPTION

As the Clinical Lead – DVA Community Nursing ,



you will play a pivotal role in the establishment, growth and ongoing success of Amazing Grace Community Services' DVA Community Nursing Program. This position combines clinical leadership, operational oversight and service development to ensure safe, effective and person-centred care for veterans within the community.

POSITION SUMMARY

The Clinical Lead will provide clinical leadership, governance and operational management of the DVA Community Nursing Service. The successful candidate will oversee the delivery of high‑quality nursing care, ensure compliance with DVA requirements and professional standards, develop referral pathways and stakeholder relationships, and contribute to the ongoing growth and sustainability of the service.

KEY RESPONSIBILITIES

- Deliver high quality nursing care to DVA clients in the community
- Liaise with GPs, hospital discharge coordinators, allied health professionals and other stakeholders
- Report assessment outcomes to the client's GP
- Develop and review individualised nursing care plans
- Delegate care to ENs and PCWs within their scope of practice
- Monitor, supervise, and support delegated staff
- Maintain accurate, up-to-date clinical documentation
- Record all delegated care in the client's clinical records
- Deliver care in line with NMBA Registered Nurse Standards for Practice
- Manage incidents and clinical risk
- Ensure continuous quality improvement
- Mentor new staff members where required
- Identify opportunities to expand services

ESSENTIAL QUALIFICATIONS & REGISTRATIONS

Mandatory





- Current unrestricted AHPRA registration as a Registered Nurse
- Previous experience as Care Manager, Clinical Manager or Clinical Care Coordinator within community sector (preferably aged care) or demonstrated senior leadership experience
- Excellent organisational and time management skills
- Community nursing experience
- Current National Police Check
- Working With Children Check
- NDIS Worker Screening Check
- Current First Aid and CPR
- Driver's licence and own vehicle
- Minimum 2 years' post-graduate nursing experience, including demonstrated wound management experience.
- Completed infection prevention and control training
- Medication competency
- Experience in Aged Care

Highly Desirable

- Grade 5 Registered Nurse (or equivalent senior clinical nursing experience).
- Experience working with DVA clients
- Wound care management experience
- Chronic disease management
- Has worked in business development and service development roles
- Minimum 5 years post-registration experience
