Employment Type: Temporary Part Time, 16 hours per week until February 2027.
Remuneration: $61.31 - $63.29 per hour + 12% Superannuation + Salary Packaging.
Location: FLYHT or The Corner.

Overview
This role provides an opportunity to make a meaningful difference by:
- Supporting young people with complex needs through comprehensive psychosocial assessment, care navigation and care coordination.
- Working within a multidisciplinary Youth Health team dedicated to improving health equity and access to services.
- Building strong partnerships with schools, community organisations and specialist services.
- Providing clinical leadership and contributing to service development and quality improvement initiatives.
- Delivering person-centred, trauma-informed care that empowers young people to achieve their health and wellbeing goals.
Qualifications and Experience
We are seeking an experienced, compassionate and highly motivated Senior Social Worker who is committed to improving health and wellbeing outcomes for vulnerable young people. The successful candidate will demonstrate:
- Extensive experience working with young people experiencing complex psychosocial, mental health and social issues.
- Strong knowledge of adolescent development, trauma-informed care and strengths-based practice.
- Exceptional assessment, care planning and clinical decision-making skills.
- Proven ability to provide high-quality care navigation and care coordination for young people with complex and multifaceted needs across health, education and community services.
- Highly developed communication and interpersonal skills, with the ability to build trusting relationships with young people, families and partner agencies.
- Ability to work collaboratively with internal and external stakeholders, including schools, community organisations and government services.
- Solid advocacy skills to support equitable access to health and community services for vulnerable young people.
- Ability to manage competing priorities, exercise sound clinical judgement and work autonomously within a dynamic community setting.
- Australian C-Class driver's license (including P1 or P2)
required. A NSW C-class license must be obtained within three months of appointment if held elsewhere.
Selection Criteria
To help us assess your suitability, please answer the following questions:
- A Bachelor or postgraduate degree in Social Work that provides eligibility for full membership to the Australian Association of Social Workers (AASW).
- Demonstrated high level clinical experience and understanding of the needs of young people aged 12 to 24 years and their families/carers accessing the health care system including Aboriginal and Torres Strait Islander, Culturally and Linguistically Diverse and refugee client groups.
- Care Navigation, Coordination and Youth Engagement – demonstrated experience in coordinating and navigating care for young people with complex health, mental health, psychosocial, education and welfare needs. Proven ability to undertake comprehensive assessments, develop care plans, facilitate referrals, advocate for clients, and engage young people and families in a collaborative, youth‑friendly manner to improve access to services and continuity of care.
- Leadership, Supervision and Service Improvement – demonstrated experience providing clinical supervision, mentoring and support to staff and students, with the ability to lead, implement and evaluate quality improvement activities, service development initiatives and collaborative models of care that improve outcomes for young people.
- Communication, Collaboration and Professional Practice – highly developed communication, interpersonal, consultation and negotiation skills, with demonstrated ability to build effective relationships across multidisciplinary teams, government and non‑government agencies, schools and community services. Proven ability to manage competing priorities, exercise sound clinical judgement and contribute to coordinated, integrated care for young people.
