Job Description

Responsibilities

- Supporting patients to attend SEWB appointments
- Assisting to coordinate SEWB Team administrative functions as required
- Conducting client outreach as requested
- Supporting and facilitating group activities
- Supporting and facilitating the thorough care of Mental Health Referrals between TAIHS GPS, the QLD Health, SEWB team and other relevant Mental Health Services
- Liaising with,



and supporting clients to access the services applicable to wellbeing
- Preparing and maintaining reports / statistics associated with the role and responsibilities
