THE ORGANISATION

As Western Australia’s first Aboriginal foster care agency, Yorganop has been working in partnership with government agencies and our community of carers across WA for more than 34 years, to provide culturally appropriate care for Aboriginal children.

In 2022, Yorganop commenced the Home Stretch WA service which supports Aboriginal young people between 18 and 21 years old, transitioning from out-of-home care to independent living.

Our over-arching purpose is nurturing children to flourish, and empowering Aboriginal children, young people and families to grow strong in themselves, their culture and their future.

Home Stretch Transition Coach to join our amazing team of dedicated coaches who are committed to walking alongside Aboriginal youth and supporting them to achieve their goals, connect with family, find stable accommodation, access employment and education opportunities, and connect with other support services.

ABOUT THE ROLE

Providing one on one individualised support to Aboriginal young people aged from 18 until they turn 21.

Reconnect or strengthen the Aboriginal young people’s connection to culture and kin.

Identify supports and services required by the Aboriginal young people and assist them to access supports under the domains of accommodation, health, identity and culture, education, training and employment.

Refer Aboriginal young people to appropriate external services and supports, as required.

Work actively with the Aboriginal young people while they are participating in the Homestretch program.

Have experience in youth work or community outreach, social work or case management

As an Aboriginal organisation, providing Home Stretch to Aboriginal young people, Aboriginal applicants are strongly encouraged .

Demonstrated knowledge and understanding of Aboriginal culture and the ability to work with Aboriginal young people, communities, and families.

Demonstrated experience in engaging and building strong relationships with and motivating ‘at risk’ Aboriginal young people.

A thorough understanding of the barriers and difficulties facing Aboriginal young people with experiences of out of home care.

Ability to be innovative and flexible, work independently, demonstrate initiative and work effectively under pressure.

Ability to assist Aboriginal young people to develop independent living skills.

Strong written and verbal communication skills.

Organisational skills to prioritise tasks and manage time effectively.

Demonstrated behaviour management, conflict resolution, advocacy and negotiation skills.

Willingness to work across the local region on regular outreach: working in the client’s home, in local community centres and any other venue where young people may require support.

Ability to work in a team and to work collaboratively with others.

Good understanding of drug and alcohol and mental health issues and support services.

Qualifications and licences





Tertiary qualification in a relevant field is preferred however unqualified applicants with a minimum of 2 years demonstrated relevant experience and skills working with young people with experiences of Out-of-Home Care will be considered.

A satisfactory National Police Clearance that must be maintained for the duration of your employment and a Department of Communities Departmental Check (Departmental Record Check & Criminal Record Check) that must be maintained for the duration of your employment.

A satisfactory Working with Children Check that must be maintained for the duration of your employment.
