Position Overview

Full‑time, Contract. Fixed Term appointment for 76 hours per fortnight with possibility of extensions and/or permanency. Initial period ends 30/06/2027.

South Metropolitan Health Service (SMHS) is seeking a Service Manager to lead the team at the Older Adult Health Hub (OAHH) in Cockburn.

Responsibilities

- Provide strategic and operational leadership for the Older Adult Health Hub.
- Manage and support a multidisciplinary team including Care Navigators, Nursing, Allied Health staff.
- Develop relationships with medical specialities, nursing, allied health, primary and community care providers.
- Refine and develop the service model to ensure it meets its core objective of providing older adults with chronic and complex conditions access to coordinated, person‑centred care.
- Oversee day‑to‑day operations, including physical infrastructure, human resources, and financial assets.
- Supervise staff, foster strategic partnerships, identify and address service gaps, and keep the Hub responsive to evolving needs of patients and primary care providers.
- Meet reporting requirements, including performance metrics, resource utilisation, and service outcomes.
- Present at the Cockburn Integrated Health and Community Facility; travel across SMHS catchment area may be required.

Eligibility
