About the role:

Are you keen to join a progressive and supportive diabetes education team? At Murrumbidgee Local Health District we are looking for a Senior Credentialled Diabetes Educator who is a proven team player. This expert clinical position is responsible for the provision of high-level, specialist diabetes education and clinical management services.

Our team is collaborative and progressive. We are developing and implementing innovative new models of care (including supported virtual services, and DESMOND) in the progression of high level education, early identification and intervention pathways.

This role aims to support staff to empower individuals with diabetes to effectively self-manage their condition, prevent complications, and optimise health outcomes. You will provide clinical leadership, education, research, and quality improvement initiatives across the health service, ensuring best practice in diabetes care within a multidisciplinary framework

You will be responsible for:

- Clinical Practice: Provide expert clinical knowledge in diabetes assessment and management, maintain currency with evidence base, act as resource and consultant to clinical teams, and collaborate with healthcare teams to develop comprehensive care plans.

- Education: Develop and deliver educational programs for staff and patients, facilitate patient and family education as partners in care, teach diabetes self-management skills,



and educate on diabetes technologies including insulin pumps and continuous glucose monitors.

- Health Systems Optimisation: Participate in multidisciplinary committees, coordinate development and review of clinical guidelines, use data to inform clinical decision-making, and collaborate with stakeholders to identify and resolve system issues.

- Research and Quality Improvement: Monitor and evaluate clinical practice, participate in research projects and clinical trials, translate research findings into practice, and lead quality improvement initiatives.

- Professional Leadership: Act as role model upholding professional conduct, provide leadership promoting positive work environments, foster continuous improvement culture, and support professional growth through mentoring and clinical supervision.

Our ideal candidate will demonstrate:

- A Tertiary Qualification with current registration/ eligibility to the relevant peak body: Accredited Practicing Dietitian (Dietitians Australia), Registered Podiatrist (AHPRA), Registered Exercise Physiologist (ESSA), Registered Physiotherapist (AHPRA) or Registered Nurse (AHPRA)

- A postgraduate qualification (Graduate Certificate or higher) in Diabetes Education and Management and current status as a Credentialled Diabetes Educator (CDE) with the Australian Diabetes Educators Association (ADEA)

- Minimum of 5 years of post-registration/qualification clinical experience, in a dedicated diabetes education and management role

- Experience in clinical leadership, mentorship, and the ability to design and deliver effective education programs
