Agentic Care Disability Services is seeking compassionate, reliable and experienced Disability Support Workers to join our team.

We are a family-led disability support provider committed to delivering respectful, person-led supports that help people exercise choice, build independence and live life on their own terms.

About the role

As a Disability Support Worker, you will provide individualised support to participants in their homes and within the community.

Depending on the participant’s needs, your responsibilities may include:

- Assisting with personal care and daily living activities
- Supporting community access and social participation
- Assisting with appointments, shopping and recreational activities
- Meal preparation and household tasks
- Medication assistance within your training and scope
- Supporting participants to work towards their individual goals
- Completing progress notes, incident reports and other required documentation
- Following participant support plans, behaviour support plans and healthcare instructions
- Promoting participant choice, dignity, independence and safety
- Communicating professionally with participants, families and the wider support team

Shifts may include weekdays, evenings, weekends, public holidays and sleepovers.

About you

We are looking for Support Workers who are dependable, respectful and genuinely care about the people they support.

To be considered for this role, you must have:

- Previous experience working in the disability sector
- A person-led and strengths-based approach
- Strong communication and relationship-building skills
- The ability to work independently and follow support plans
- Good written documentation and progress-note skills
- Reliability, professionalism and a positive attitude
- The ability to maintain appropriate professional boundaries
- Confidence supporting people with a range of disabilities and support needs

Essential requirements

Successful applicants must hold:

- A current NDIS Worker Screening Check




- A current Working with Children Check, where required
- Current First Aid and CPR certificates
- A current National Police Check
- A valid Australian driver licence
- Access to a reliable, registered and insured vehicle
- Current right to work in Australia
- NDIS Worker Orientation Module certificate
- Infection prevention and control training
- Certificate III or IV in Individual Support, Disability, Community Services or a related qualification, or equivalent relevant experience

Experience or training in complex supports, manual handling, medication assistance, positive behaviour support, mental health or high-intensity supports will be highly regarded.
