Phoenix Horizon is seeking a compassionate, reliable and experienced Female Disability Support Worker to support a female NDIS participant located in Narre Warren.

This is an ongoing opportunity with consistent hours for the right applicant.

Shifts Available

- 5–6 hours per day
- Ongoing weekly roster
- Immediate commencement

About the Role

You will provide high-quality, person-centred support, assisting the participant to live safely, independently and with dignity in her home and community.

Key Responsibilities

- Personal care (showering, dressing, grooming and hygiene)
- Assistance with mobility and transfers (if required)
- Meal preparation
- Light domestic assistance
- Medication assistance (where applicable)
- Community access and appointments
- Social support and companionship
- Maintaining professional progress notes and documentation
- Following the participant’s support plan
- Promoting independence, choice and control

Essential Requirements

- Female support worker (participant preference)
- Experience providing personal care
- Current NDIS Worker Screening Check




- Current Working with Children Check (Employee)
- Current National Police Check
- Current First Aid & CPR Certificate
- NDIS Worker Orientation Module Certificate
- Valid Victorian Driver’s Licence
- Reliable and fully insured vehicle
- Comprehensive car insurance
- Current ABN (preferred)
- Current Public Liability Insurance (if contracting)

Desirable

- Certificate III or IV in Individual Support, Disability, Aged Care or equivalent
- Experience supporting participants with high or complex support needs
- Medication Administration competency
- Manual Handling experience
- Knowledge of the NDIS Practice Standards and NDIS Code of Conduct
