Outreach Mental Health Support Worker

About the Role:
Conscious Care Support is seeking dedicated Outreach Mental Health Support Workers to provide psychosocial, emotional, and practical support to individuals with chronic psychosocial disabilities. In this role, you’ll:
✔ Work independently in clients’ homes and the community
✔ Support individuals in identifying and achieving their goals
✔ Collaborate with care teams for high-quality, client-centred support.

This is an outreach role, meaning you’ll work autonomously while still being part of a supportive, tight-knit team.

We are looking for individuals with at least 3–4 days of weekday availability. While we offer weekend services, most support is provided Monday to Friday.

What You Need:
✔ Cert IV in Mental Health (minimum)
✔ Strong case note writing skills (concise & articulate)
✔ In-depth understanding of mental health and psychosocial disability
✔ A genuine passion for empowering others
✔ A driver’s license & comprehensively insured vehicle
✔ A mobile phone for communication with clients and stakeholders

Additional Requirements:
✔ NDIS Workers Screening Check
✔ Police Check (from within the last 12 months)
✔ Working with Children Check (WWCC)
