Clinical Care Manager
A rare leadership opportunity has become available for an experienced and passionate Clinical Care Manager (CCM) to join Aeralife Twin Parks.
As a key member of the Residence Leadership Team, you will provide clinical leadership across the home, driving quality care outcomes, supporting and developing clinical staff, and ensuring residents receive safe, person-centred care that enhances their wellbeing and quality of life.
Working in partnership with the Residence Manager, you will play a pivotal role in fostering a culture of clinical excellence, continuous improvement, and regulatory compliance while contributing to the ongoing success and growth of our community.

The Opportunity
As the Clinical Care Manager, you will be responsible for leading all clinical operations within the residence and ensuring residents receive exceptional, individualised care that meets their unique needs, preferences and goals.
You will provide clinical oversight, mentor and support nursing and care teams, drive continuous quality improvement initiatives, and ensure compliance with the Aged Care Quality Standards and relevant legislative requirements.
Key Responsibilities
- Provide leadership and oversight of all clinical care services within the residence
- Ensure care delivery is person-centred, contemporary and aligned with residents' individual needs and preferences
- Partner with residents, families, representatives and healthcare professionals to support optimal health outcomes
- Lead, coach and develop clinical and care teams to achieve excellence in care delivery
- Monitor and improve key clinical indicators and quality outcomes
- Ensure accurate clinical assessment, care planning, documentation and evaluation processes
- Drive continuous improvement and maintain compliance with accreditation, quality and regulatory requirements
- Promote a culture of accountability, collaboration and clinical excellence
- Build strong relationships with GPs, allied health professionals and external stakeholders
About You
You are an experienced aged care clinician with a passion for leadership, quality care and achieving outstanding resident outcomes.
To be successful in this role, you will have:
- Current unrestricted registration as a Registered Nurse with AHPRA
- Minimum three years' experience in a senior clinical leadership role within residential aged care
- Strong understanding of the Aged Care Quality Standards and contemporary aged care legislation
- Demonstrated experience managing clinical governance, compliance and quality systems
- Excellent clinical assessment and care planning skills
- Strong leadership, coaching and team development capabilities
- Sound analytical, problem-solving and decision-making skills
- Proficiency in electronic clinical management systems and reporting platforms
- Exceptional written and verbal communication skills
- Current NDIS Worker Screening Check (or ability to obtain)
- Current COVID-19 vaccination in line with industry recommendations
