- Salary Range: $88,464.00 - $115,289.20 per annum, pro rata, + superannuation
About the role
South West Healthcare (SWH) is seeking applications for a rewarding opportunity to join our Discharge Support & Liaison team! If you're a dedicated Registered Nurse, Occupational Therapist, Physiotherapist, Social Worker, Speech Pathologist, Podiatrist, or Dietitian passionate about supporting patients with timely and safe hospital discharge, we want to hear from you.
As a valued member of our team, you will collaborate with a multidisciplinary team to enhance patient flow, deliver optimal care, and reduce hospital re-admissions. This role is your chance to play an essential part in ensuring patients receive the right care, at the right time, in the right place.
What you bring
- Minimum of 3 years' experience in your health profession
- Creativity, flexibility, and the ability to adapt to organisational changes
- Strong time management, organisation, and problem-solving skills
- The ability to work independently while contributing to a multidisciplinary team
- Excellent interpersonal and communication skills
- Experience in subacute and community settings (desirable but not essential)
