Post-Acute Care (PAC) Program Care Coordinator

District Nursing & Continuing and Complex Care

Permanent Part Time, up to 40 hours per fortnight

Nurses and Midwives Agreement, HITH/PAC Nurse Level 1 ($58.34474 p/h, excludes allowances)

An exciting opportunity exists to join the Continuing & Complex Care team at West Gippsland Healthcare Group as a Post-Acute Care (PAC) Program Care Coordinator. In this rewarding role, you will coordinate short-term community-based services that support patients transitioning from hospital to home, helping them recover safely, maintain independence and avoid unplanned hospital readmissions.

Reporting to the District Nursing Service & Continuing and Complex Care Nurse Unit Manager, you will work collaboratively with patients, families, hospital staff, community providers and other healthcare professionals to assess needs, coordinate services and ensure effective continuity of care.

This position would suit a motivated and experienced Registered Nurse who enjoys autonomous practice, has solid clinical assessment skills and is passionate about supporting positive patient outcomes in the community setting. Our preferred candidate will have:

- Current AHPRA registration as a Registered Nurse.
- Demonstrated experience and knowledge of Community Services, District Nursing and Acute Care services.
- Knowledge of Health Independence Programs, particularly the Post-Acute Care (PAC) funding model.
- Strong clinical assessment skills, including the ability to conduct comprehensive phone assessments and determine appropriate care pathways.
- Excellent communication, stakeholder engagement and relationship management skills.
- Proven ability to work independently and effectively within a multidisciplinary team.




- Strong organisational and time management skills with the ability to manage competing priorities.
- High level computer literacy and experience using electronic information systems.

The following experience/qualifications would be considered advantageous:

- Previous experience within a hospital or healthcare environment.
- Experience with program administration, data collection and reporting.
- Proficiency in Microsoft Office applications, particularly Word and Excel.

IMPORTANT: Some roles in the healthcare sector are required to be vaccinated annually for Influenza (unless a valid exemption applies). Evidence of vaccination (or exemption) will be required to be provided prior to commencement.
