Disability Support Coordination involves assisting NDIS participants to understand, implement, and manage their NDIS plans. The role focuses on coordinating supports, services, and stakeholders to assist participants to work towards their goals.

This is a work-from-home position. Residence within the Geelong region is required due to the expectation of face-to-face engagement with participants when required.

We are currently seeking experienced coordinators who have experience sourcing referrals, and who would be willing to support with our current case load.

Our KPI for this role is to maintain a 75% billable rate. Hours can be flexible, ranging from 16-38 hours per week, provided the KPI can be met.

Job Requirements and Responsibilities

Requirements

- Experience working within the NDIS framework, including plan implementation and coordination of supports

- Experience working with people with disability in community or support settings

- Understanding of person-centred and strengths-based practice

- Ability to manage a caseload and work independently

- Strong written communication skills, including preparation of reports and documentation

- Strong verbal communication and stakeholder coordination skills

- Ability to work with multiple service providers and complex support arrangements

- Ability to advocate within service systems

- Strong organisational, analytical, and problem-solving skills

Responsibilities

- Provide support coordination services within the NDIS framework

- Assist participants to implement and manage their NDIS plans

- Coordinate formal and informal supports aligned with participant goals

- Develop, implement, monitor, and review participant support plans

- Liaise with participants, families, service providers, and other stakeholders

- Facilitate communication across all parties involved in participant supports

- Support participants to exercise choice and control in service delivery





- Maintain relationships with NDIS providers, community organisations, and allied health professionals

- Maintain accurate participant records, plans, and budgets using Astalty software

- Ensure compliance with NDIS Price Guide and relevant operational guidelines

- Review and adjust supports in consultation with participants and stakeholders

Working Workplace

Support Coordinators will primarily work from home. You must have a suitable environment (including an office space and adequate internet) to complete this role successfully. You may also be required to visit participants in their home, therefore a valid driver’s license and roadworthy car are also a requirement.

Remuneration

This role falls under the Community Access Stream of the SCHADS Award. This role has been classified as level 4 under this award. Level 5 may be available for candidates with a higher level of experience.

Essential Qualifications

- NDIS Worker Screening Check

- Working with Children Check

- Valid Australian Driver’s Licence and roadworthy vehicle

- First Aid Certificate

Desirable Experience and Qualifications

- Previous experience in NDIS Support Coordination (Level 1 or Level 2)

- Experience working with complex participant needs and multiple support providers

- Familiarity with Astalty or similar case management systems

- Established networks within the NDIS, disability, health, or community services sectors

- Experience with service navigation, referrals, and coordination across multiple systems

- Experience in negotiating supports, resolving service issues, and managing funding-related queries

- Experience working with a diverse range of participant groups, including psychosocial disability, intellectual disability, and physical disability

- Additional qualifications in disability, mental health, social work, community services, or related disciplines

- Knowledge of allied health systems and community-based supports

- Experience working in roles requiring independent caseload management
