Role Description: To assist with in-home nursing needs and care of clients in the community. The role is ideal for a competent Nurse who wants flexible schedule on a casual basis with a passion for the community's best interest. 2-hour minimum shift as per Nurses Award.

REQUIREMENTS
Degree in Nursing with current registration with AHPRA
Maintain a current National Police Check (within 3 years)
Driver’s license as travelling may be required to assist the clients around Illawarra
Current Working with Children Check
Current First Aid & CPR
Adhering to the relevant policies and procedures
General understanding of the health and aged care system.
Computer skills

Duties:

The role will require assistance with low to high care including but not limited to
wound care, catheter care, assessment and plan reviews, assisting with Medication management and maintaining accurate records, and applying qualified assessment skills to identify the physical, mental and psychosocial needs of clients and being able to develop a care plan to meet these needs using a multi-disciplinary approach when required




Ability to communicate effectively with patients, families, physicians, case managers and hospital staff
Assistance with activities of daily living

Demonstrated empathy with elderly and disability participants.
Positive interpersonal skills and holistic assessment skills.
Good time management skills, flexibility and an ability to set goals; prioritise tasks and work under pressure.

Complete handover notes along with any completed forms for further reviews
Advocate for clients to ensure best outcomes.
