Responsibilities
Assessment and Care Planning: complete core clinical tasks such as assessments, reviews, referrals, meetings (HOD/MDT/case conferences) and staff education as part of the multidisciplinary team on site.
Manual Handling Training: deliver practical, hands‑on and engaging sessions that challenge participants to complete tasks and demonstrate their knowledge of manual handling.
Wellness and Reablement Programs: deliver individually tailored programmes aligned with each resident’s goals and needs.
Home Visits – Community Physiotherapy: support people in their homes and in the community to maintain independence and function.
Duties
Assess, plan and deliver evidence‑based, goal‑oriented, and outcome‑driven therapy.
Build a vibrant wellness culture that enhances resident outcomes and engagement.
Deliver wellness and reablement programmes through 1:1s and group programmes.
Build robust client relationships.
Document clearly and efficiently.
Collaborate with clinical & multidisciplinary teams.

AHPRA registration.
National Police Check.
