Hesse is seeking an aged care in office pharmacist (ACOP) to join a multidisciplinary team in a residential aged care facility. The ACOP will manage medication reviews and pharmacotherapy, working closely with GPs, in office nurses, and facility staff to improve resident health outcomes.

This is a contractor, part time position (2 days per week) emphasizing integrated care and quality use of medicines. Credentialing required in ACOP and Medication Management reviews, and ABN holder status is
