We are seeking a Support Coordinator to join our Care Connect team on a Part Time, 3 days per week basis, working from the Forster Neighbourhood Centre.

Our Care Connect program is an aftercare service, supporting people experiencing a suicidal crisis or who have attempted suicide. The service focuses on empowering people to connect with supports in their recovery.

What you'll be doing
Provide non-clinical, psychosocial support to people who have been discharged from hospital after a suicidal crisis or attempt
Support participants to develop personal safety plans
Support participants to explore their goals,
and engage with other relevant clinical, community and local services
Support participants with building a solid therapeutic alliance based on their identified needs
What we're looking for
Diploma or Cert IV (minimum) qualification in Mental Health or Community Services
At least 2 years’ experience in a suicide prevention /aftercare or mental health setting.
Experience providing person‑centred support, safety planning and case management with people experiencing suicide ideation
