Centrecare is recruiting a Homelessness Services Connector to triage clients and refer them to accommodation and support services across the Perth Metro, Goldfields and Bunbury region.
You will liaise with specialist homelessness services, update the Specialist Homelessness Information Platform, and communicate referral outcomes to clients using an online tool. Knowledge of WA service networks is essential.
