Job responsibilities: Provide patient and family education related to the plan of care and discharge needs.
Support protected transitions of care, including handoffs and care plan updates.
Work 36 hours per week with 12-hour shifts.
Maintain infection prevention practices and safe medication administration standards.
Perform focused reassessments and escalate concerns through appropriate clinical channels.
