You will play a key role in supporting consumers experiencing complex mental illness by providing comprehensive assessment, crisis response, therapeutic interventions and sustained case management in a trauma‐informed, person‐centred framework.

Your Clinical ImpactAs a senior community clinician, you will use advanced mental health nursing or allied health skills to:
Provide high‐qualitypsychiatric assessment, formulation and risk stratification, including suicide and violence risk.
Leadrecovery‐oriented case management, supporting consumers with complex biopsychosocial needs.
Deliverstructured therapeutic interventionssuch as CBT‐based strategies, safety planning, relapse prevention, sensory modulation, psychoeducation and family‐inclusive practice.
Conductcrisis assessmentsin the community, ED, home‐based care or telehealth environments.
UndertakeTriage & Intake assessmentsusing clinical prioritisation frameworks and assertive follow‐up.
Partner with HOPE or post‐discharge teams to provide assertive outreach, engagement and ongoing safety planning.
Collaborate with GPs, psychiatrists, AOD services, NDIS providers, housing, and other community agencies to ensure integrated care.
Drive clinical governance through participation in clinical reviews, reflective practice, care planning meetings and multidisciplinary case conferences.




Key Responsibilities:
Conduct comprehensive mental health assessments including MSE, risk formulation, diagnostic considerations and needs assessment.
Develop and implement individualisedtreatment, support and recovery plans.
Providecrisis intervention, including short‐term intensive support and escalation where required.
Deliverevidence‐based therapeutic interventions, both individual and group‐based.
Coordinate care across multiple service providers and promote multi‐agency collaboration.
Maintain high‐quality clinical documentation aligned with the Mental Health & Wellbeing Act ****.
Participate actively in multidisciplinary team meetings, clinical governance, and ongoing service development.
Contribute to quality improvement, service innovation and best‐practice care pathways.
Key Requirements:
Minimum 2+ yearsrecent experience in Community Mental Health (public mental health preferred).
Tertiary qualification in Nursing, Social Work, Psychology or Occupational Therapy withcurrent AHPRA or AASW registration.
Demonstrated expertise in:
Complex risk assessment & crisis response
Care coordination & recovery‐oriented practice
Trauma‐informed, culturally safe and consumer‐led care
Robust understanding of theMental Health & Wellbeing Act ****, recovery model, and Victorian public mental health frameworks.
Ability to work autonomously and collaboratively in a multidisciplinary environment.
Excellent interpersonal, written and verbal communication skills.
Valid driver's licence (community work required).
