About the role:
Western Health is seeking an experienced and compassionate clinician to join ourEmergency Department Care Coordination (EDCC) Teamon a12-month fixed-term, full-time basis.
This is an exciting opportunity to work within a dynamic, multidisciplinary service that supports some of the most vulnerable members of our community by delivering innovative, person-centred care within the Emergency Department.
As anEmergency Department Care Coordinator, you will play a key role in improving patient outcomes by reducing avoidable hospital admissions, facilitating safe and timely discharges, and coordinating care for patients with complex medical, psychological and social needs.
This role has a strong focus on theFrequent Presenter Portfolio (FPP), supporting patients who regularly attend Western Health Emergency Departments through the development of individualised, trauma-informed management plans that promote consistent, safe and effective care.
Key responsibilities:
Working within a transdisciplinary team, you will provide comprehensive care coordination for patients presenting with complex needs, including those experiencing:
Substance use issues
Homelessness or housing insecurity
Disability
Family violence
Complex aged care needs
Neurodiversity and behaviours of concern
You will collaborate closely with emergency clinicians, inpatient teams, community providers and external agencies to remove barriers to discharge, improve continuity of care and support patients to access appropriate services within the community.
As part of the Frequent Presenter Portfolio, you will:
Develop and coordinate multidisciplinary Emergency Department management plans for patients with complex health, psychosocial and behavioural needs.
Work collaboratively with internal and external stakeholders to ensure a consistent, coordinated approach to patient care.
Apply trauma-informed, recovery-oriented and person-centred principles to improve patient outcomes.
Identify patient triggers, communication preferences and de-escalation strategies to support both patient and staff safety.
Contribute to initiatives that improve patient flow, reduce avoidable Emergency Department presentations and support timely access to appropriate services.
About you:




You are a skilled clinician who thrives in a fast-paced environment and enjoys working collaboratively to solve complex problems.
You have a genuine commitment to person-centred care and are passionate about improving outcomes for vulnerable populations.
You will bring:
Qualifications in Nursing, Social Work, Occupational Therapy, Physiotherapy
Current registration with the relevant professional board or association.
Demonstrated experience working with people experiencing complex health and psychosocial needs.
Strong clinical assessment, care coordination and discharge planning skills.
Experience working within multidisciplinary teams.
Excellent communication, negotiation and stakeholder engagement skills.
A trauma-informed, recovery-focused and culturally safe approach to care.
The ability to think critically, prioritise effectively and work autonomously in a busy Emergency Department environment.
Experience in Emergency, Acute Care, Mental Health, Community Health or Care Coordination will be highly regarded.
