What you will do
Manage a varied caseload of adults receiving support from secondary mental health services, delivering person-centred interventions and ongoing case management.
Complete strengths-based assessments, develop care and support plans, and regularly review outcomes to promote independence and recovery.




Work closely with the Transition Team, supporting young people moving into adult mental health services.
Experience within Children's Social Care is desirable but not essential.
Collaborate with service users, families, healthcare professionals, and partner agencies to provide coordinated, holistic care.
Maintain accurate, timely, and legally compliant case records and documentation in line with statutory requirements and qualified standards.
Requirements
Qualified Social Worker with currentSocial Work Englandregistration.
Experience working withinAdult Social Care, ideally in a Community Mental Health Team or similar setting.
Strong understanding of theCare Act ****,Mental Health Act,Mental Capacity Act, and related legislation.
Excellent assessment, care planning, communication, and multidisciplinary working skills.
Ability to manage a busy caseload while delivering high-quality, person-centred practice.
