$107K + super + Aveo vehicle (with fuel card)
The Opportunity
As a
Care Partner (Clinical)
, you'll be more than a Registered Nurse - you'll be an advocate, a familiar face, and a trusted partner to a caseload of Aveo residents as they navigate their home care options. Immersed within our Aveo retirement communities, you will spend your day in residents' homes building genuine connections, explaining care options, creating personalised plans and coordinating services to ensure that Aveo residents continue to live independently for as long as possible.
Your Impact
Meet with prospective residents and families to assess care needs and explain funding options clearly and compassionately
Develop personalised home care plans and ensure services are in place as required
Coordinate with external service providers including GPs, Allied Health and other care and support services
Monitor resident wellbeing over time and adjust care plans as needs evolve
Guide residents and families through the My Aged Care and Support at Home journey, providing excellent customer service at every moment
Maintain accurate documentation and records via our Care Management System (Alayacare)




Promote Aveo Care at Home services by attending community events and delivering information sessions to residents
What you'll bring
Current Registered Nurse AHPRA registration
Prior experience in the home care or community care industry (e.g. Case Manager or Care Coordinator)
Practical knowledge of Support at Home (or HCP) program and/or My Aged Care and ACAT assessments
Confidence using clinical judgement in an autonomous, community-based setting
Excellent communication skills — with residents, families, and multidisciplinary teams
Confident using a Care Management System (e.g. Alayacare or similar), Microsoft Office and proficient with mobile technology
Drivers License and two (2) COVID Vaccinations (required)
