As the Care Manager Team Leader, you will support and supervise Care Managers to deliver high-quality, relationship-centred community-based services.
Reporting to the Managing Director, you will help ensure care management practice is consistent, compliant and aligned with legislative, regulatory and organisational requirements.

Key Responsibilities
Support consistent delivery of high-quality, relationship-centred services across the client journey.
Provide day-to-day supervision and guidance to Care Managers across care planning, documentation, risk monitoring and service coordination.
Act as the first point of escalation for complex client matters, safeguarding concerns and service risks.




Conduct regular audits of care plans, client reviews, budgets, risk assessments and documentation to maintain compliance and audit readiness.
Lead capability development through onboarding, mentoring, performance support and targeted education for Care Managers.
About you
A tertiary qualification in Nursing, Allied Health, Social Work or a related discipline will be highly regarded.
Demonstrated experience leading and developing high-performing teams within aged care, community services or a similar setting.
Confident working across quality, compliance, risk mitigation, documentation and continuous improvement requirements.
Strong analysis, problem-solving, organisational, time management and accurate data entry skills.
Ethical, self-motivated and collaborative, with a values-led, person-centred approach to service delivery.
